Happened for me when dragging my turboprint drawer into ram: this one isn't in text mode. nor are any subdirectories.
I noticed the "false" uicon filckered in and out as copying proceded, and closing and reopening the ram: disk removed it.

I got the same 'extra icon' when copying files to a USB thumb drive.

Here the fake icon appears when any copying takes place (not just to/from ram:). The fake icon always appears in the root of the target drawer's partition (and not in the target drawer itself). The icon appears in any view, as long as "All files" are shown.

I don't know about all these particular settings,
But I dragged a directory icon from another partition (with some files in it) into the ram disk of a freshly installed 4.1.3 installation and the ghost icon appeared... I hadn't changed anything in the sys config so everything must be factory set already?

I can confirm this, too. Furthermore this only happens if "Update drawers automatically" is activated in Workbench prefs. So it has to relate to this relatively young (but great) feature.

This has subsequently been fixed internally, and should reach end users in a future update.

One interesting thing is that it happens also in other places. For example I moved my gNgeo drawer from a back-up DVD to DH4: ( DH4: it's a JXF4 partition I named "Downloads"):

The strange thing is that if you double click this strange nameless drawer nothing happens (in another occasion double clicking on such a drawer happened to close the window though) but if you try to delete it, it deletes the contents of the partition!
I had MUIOWB there which got wiped out o_O (anyway reinstalling it solved a problem I had with fonts parsing at every launch).
I came here to report this too and found out there was already this thread about it.
P.S
The gNgeo drawer is snapped in icon mode by the way not text.

Not here. If I have the source set to "Show = All files" and "View by = Icon" I get the same double icon effect in the destination drawer.
