I think the key here is to use the Mixer (in Utilities I believe) to set the volume of your sound card. Making sure all AHI units, including the Music unit, have a decent volume configured (like medium or max depending on your stereo setup) might help as well. The narrator.device will probably be diverted to the Music unit to allow to get mixed with other sounds even if it would rather like to have exclusive access to the sound.
