They are not "extra". You should edit the original 1440×900 mode line - first try with +RB, then if that doesn't work, try with -RB instead. But don't leave any other 1440×900 mode while you try it.

Thanks for the tip. I tried both combinations but the result was the same. Those white strips keep popping up. I find it really strange that the card works fine under OS3.9 but not on OS4.1. From what I can read there and elsewhere it seems like the most common issue is to find a card with the correct voltage that works with the Mediator.
Try / check the tooltype, the following example is of my Radeon 9250 pci on the Sam440; in particolar i observated the Interrupt tooltype difference on the bvision of Classic 1200 the interrupt=No , on the Radeon 9250 in the Sam it's Yes :

I tired changing the interrupts to on or off and those other parameters that you showed in the screenshot like CMPLENGTH to 11. But it gives the same result. The screen gets disturbed with some stripes after a short period. I think I'll have to give up on this. It must be a driver issue and I don't see any working solution for a Mediator system. The only thing I can get to work is with a Voodoo card but that's before Update 1. After update 1 that card stopped working.

Have you got the WAIT jumper on or off on the Mediator ?
What is the Memory Window Jumper set to ?
Is the Radeon in the top PCI slot, have you tried it with all other cards removed ?
Do you have a picasso96settings files in devs: which can override the Monitor Tool Types ?
