After a bit of research it looks as if my card is a relabeled

3COM 3CRDW696

If anyone would like to confirm that.

I do know that a company with a ridiculous name (Sequoia - I looked it up) had a load of 3Com cards going cheap. I bought one, and mentioned them on an Amiga forum in case anybody else was interested. At the time, AmigaKit didn't have any in stock, but they did shortly afterwards... hmm...

So, it wouldn't surprise me if they are that model, but I imagine AmigaKit get hold of whatever Prism2 cards they can and sell them under the same product code - somebody who bought one earlier, or later than you might well have a completely different model.

Why do you need to know? If it's for a firmware update I had terrible trouble finding working Windows drivers for mine* (you have to use the proper 3Com ones or it doesn't get recognised - you might be able to hack the subsys stuff out of the .inf file of a different brand, but I didn't try that)

Alternatively, you could try a MAC address lookup. It might just show up as Intersil though, not sure, don't have the MAC of my card here to check.

* It probably came with the correct ones on a CD, but I couldn't find the CD.
